Below we have discussed about the different types of bladder cancer in brief:

Transitional cell carcinomas:
90% of the cases of bladder cancer are transitional cell carcinomas. Patients with this bladder cancer type develop cancer in the transitional cells; these are the cells forming the lining of the bladder wall. One of the subtypes of transitional cell carcinomas is termed as papillary carcinoma. Papillary carcinoma is a mild bladder cancer type; it grows superficially. The other subtype of this bladder cancer type is sessile tumor; this subtype is more severe in nature and usually spreads to other body parts even during the early stages of the cancer. This makes the prognosis of sessile tumor much worse compared to that of papillary carcinoma.

Squamous cell carcinomas:
This type of bladder cancer is not as common as the transitional cell carcinomas. According to current statistics offered by NCI, 8% of the cases of bladder cancer are diagnosed to be squamous cell carcinomas. The squamous cells also form the lining of our bladder. According to medical experts, this bladder cancer type is more common among patients, who have suffered from parasitic bladder infestations in the past. Parasitic bladder infestations are marked by chronic irritation in bladder.

Adenocarcinoma:
Only 1% of the patients suffering from bladder cancer are diagnosed to have this bladder cancer type. The adenocarcinomas start from the gland cells of the patient’s bladder. Usually, adenocarcinoma is more severe compared to the two bladder cancer types discussed above. Almost every case of adenocarcinoma is invasive in nature i.e. they invade the bladder through its walls and reach other organs and tissues of the patient’s body.

Other than these 3 main bladder cancer types, one might also develop bladder cancer types like sarcomas and small-cell carcinomas. However, these cancer types are extremely rare and together accounts for only 1% of the cases of bladder cancer.

According to recently obtained facts, in the US, around 40,000 men get diagnosed to be suffering from bladder cancer every year. One becomes victim of this cancer type when cancerous cells start growing along the bladder’s wall. Here, we will be discussing about the signs of bladder cancer experienced by male patients:

Blood in urine:
Men suffering from bladder cancer might notice blood in their urine. This symptom is medically referred to as hematuria. The urine containing blood in it might appear abnormally cloudy or might be extremely dark in color (red, brown or amber). In some cases, blood in urine can be detected only by a pathologist while examining the urine in the laboratory.

Painful or frequent urination:
The cancerous cells present in the bladder might result in irritation and inflammation in the organ. The inflamed and irritated bladder might make the affected man experience urgent need of urinating pretty frequently. This symptom is often accompanied by burning sensation and pain during urination. Men suffering from bladder cancer might also develop urinary tract infections, which end up aggravating the other signs of the cancer.

Abdominal and back pain:
We have mentioned above that growth of cancerous cells in the bladder results in irritation and inflammation of the organ. This inflammation might result in severe stomach pain. Majority of the men suffering from bladder cancer experience a persistent aching pain in their lower abdomen; this pain may also radiate towards the patient’s lower back. A constant sharp pain in the lower back is one of the common indicators of bladder cancer in men. The sharp pain might also be a symptom of infection in the bladder that has already reached the patient’s kidneys; such infections also require immediate medical attention.

The year 2010 witnessed over 70,000 new cases of bladder cancer. This increase in the number of patient lisuffering from bladder cancer has forced common people to look for ways of preventing the onset of bladder cancer. According to medical experts, eating the right foods might help us in avoiding the occurrence of this disease. Patients with bladder cancer must include the following dietary instructions.

Eat selenium rich foods : Some studies have shown that consuming food containing high levels of selenium might reduce one’s risk of getting bladder cancer up to a great extent. The mineral selenium plays the role of antioxidants and thus reduces cell damages occurring within our body. There are also studies that have revealed links between selenium deficiency and increased susceptibility towards getting bladder cancer. Some of the foods items containing high amounts of selenium are: walnuts, oatmeal, eggs, cod, ground beef, poultry, Brazil nuts etc.

Drink enough fluids : It has been found that people consuming different varies of fluid throughout the day are less at risk of getting this form of cancer. However, the most effective liquid for preventing bladder cancer is water. Doctors recommend drinking a minimum of 12 cups of water every day to avoid occurrence of bladder cancer.

Add a lot of fresh vegetables and fruits to your diet : According to medical studies, adding vegetables and fruits like spinach, broccoli, kale, mustard greens, oranges, nectarines, kiwi, cherries, strawberries etc to your daily diet helps in preventing bladder cancer. Vegetables like radishes, beets, potatoes and cauliflower are also known to reduce the risk of bladder cancer.

Drink green tea regularly : One of the most useful constituents of green tea is epigallocatechin gallate, an agent also often referred to as EGCG. Consuming 2 cups of green tea daily helps in preventing bladder cancer. Drinking green tea is also beneficial for patients who are already diagnosed with bladder cancer; it will increase their survival rate significantly.

Some cancer patients might be keen to find out the survival rates of their disease form while some might not feel that the figures are of any help or might also not be interested in knowing about them. Whether one wants to know about the survival rates is basically a personal decision.

The 5-year survival rate is the proportion of cancer patients who live a minimum of five years following cancer diagnosis. Undeniably, several individuals managed to survive well beyond five years. The 5-year comparative survival rates contrast the numbers of individuals that are yet living five years following their cancer identification to the survival rates of other people from the analogous age group that have no cancerous presence. It is a more superior means of seeing the effect which cancer could have on survivals.

Bladder Cancer Survival Rates according to grade & stage

Similar to several other cancer forms, the bladder cancer survival rates are based on how much the disease had metastasized (cancer stage) during diagnosis. Cancer grade even holds importance. Cancer is categorized into three groups based on cell appearance – low, moderate & high /grades 1,2,3 with G3 being high grade (cancerous cells with maximum abnormality & utmost likelihood of quick growth & metastasis.

From all bladder cancer cases from Wales, England, fifty-seven from every one hundred males survive for a minimum of five years following their diagnosis. Forty-seven from every one hundred females would survive for a minimum of five years. The disparity amid survivals in females & males is believed to be because of the slight difference in bladder make-up. Such figures are relative survival stats that translate to the fact that they exclude individuals dying due to other reasons. Cancer stats are mostly chalked out in this manner since it offers a far precise representation of cancer survival rate. Several cancer patients are in the old age bracket & might not be dying from cancer, however from other sicknesses like cardiovascular ailment.

Early Bladder Cancer

Largely, diagnosis of bladder cancers are done when they are yet limited to the bladder lining. Amid eighty to ninety from a hundred individuals having such bladder cancer staging could survive for over five years.

Around seven from a hundred bladder cancers are of such an early form during diagnosis known as Ta (T1) bladder carcinoma in situ (CIS/Tis).

Cure of such early bladder cancer cases are mostly doable or brought under control with minor surgical intervention or therapy into bladder. Grade 1(low-grade) early bladder cancers have a greater cure rate while Grade 2, 3 (medium, high) bladder cancers have a greater likelihood of recurring & needing additional therapy.

When all superficially located bladder cancers are taken in unison, around fifty to seventy percent of them would recur, though majority of them would recur as early, non-invasive disease. Around ten to thirty percent of them would recur as cancer which has metastasized into the deep-set bladder layers.

Majority of the Ta cancers are low-grade and around half of them would recur post-therapy, though majority of them would recur as superficially located tumors which could be surgically excised with ease. Less than ten percent of them would recur as invasive cancer requiring more rigorous therapy.

Largely, T1 early bladder tumors are high grading and have a greater likelihood of forming invasive cancer. Nearly fifty percent would turn invasive when untreated. Hence, there is a greater chance of that patient needing additional therapy done to the bladder following removal of such bladder cancer.

Invasive Bladder Cancer

Approximately thirty percent of bladder tumors have permeated the bladder’s muscle sheet and are known as invasive bladder cancers. Around half of the T2 invasive bladder tumors identified among people survive three years following diagnoses. Regrettably, therapy meets with lesser success among cancers which are deep-set into the bladder wall. Around twenty-five percent of individuals having T3 bladder cancer would be surviving three years past diagnoses & therapy.

In the United Kingdom, experts deploy surgical intervention for bladder removal or radiation therapy which appears to be working analogously well. The option is mostly based on personal preference & what the expert suggests.

Surgical intervention or radiation therapy would treat a number of such cancers, however among several cancer patients the disease would relapse & necessitating further therapy. Physicians generally wouldn’t be capable of telling what cancers would possibly be recurring & what ones have a possibility of being treated. The extent of metastasis into bladder wall is among the highly vital factors. For example, staging T(3,4) have a greater likelihood of recurring as compared to T2 staging tumor. In case cancer has metastasized inside the lymph glands then the risk of recurrence is significantly more.

To lower chances of cancer recurring, then most probably chemotherapies would be given intravenously alongside radiation therapy or surgical intervention. During many large-scaled scientific studies, chemo pre-surgery reduced the chances of relapse in higher-risk bladder cancers & enhanced results to a slight extent.

Metastatic Bladder Cancer

Also known as advanced bladder cancer, the disease has metastasized to other regions of the body. Around one from twenty bladder cancer has shown metastasis during diagnosis. The prognosis isn’t favourable as is the case in early staging of disease. In case the cancer has merely metastasized to the lymph nodes near the bladder, & removal of nodes done at the time of surgical intervention then around three from ten individuals survive past five years. When cancer metastasized to lymph nodes in other regions of the body, amid ten & fifteen percent of individuals would be surviving beyond five years.

Regrettably, this form of cancer is unlikely to be treated when it has reached organs like liver, bone and lung. Yet, one would need therapy for controlling it & aid in controlling symptoms one may experience. When cancer has metastasized in this manner, on an avg. individuals would be living amid twelve to eighteen months. However, it is crucial to bear in mind that such information is avg. & several individuals would have a lengthier existence than this.

The illness of a bladder cancer can be seen in the urinary bladder, in the form of a critical growth in the sensitive tissues. In this condition, the bladder fails to manage theses kinds of abnormal growth. Bladder is a strong organ, which has the power to collect and hold urine. The bladder is situated in the pelvis. Physical experts have found that the cancer development in bladder begins with the cell that is has given a thin cover inside the bladder. This cell is known as transitional cell carcinoma. Many researchers are also working to find new methods of treatment for a bladder cancer.

It has been observed that the person who smokes a lot can have cancer in the bladder. In addition, the people who are working in exploitive conditions, like those employees of chemical factories, textiles, paints and printing industry or the people who have very less nutrient diet can suffer with the issue. A bladder cancer can also be observed in case of a birth defect in the bladder or if a person has a continual inflammation or soreness in the areas of bladder. Some sort of bladder stones can also cause a cancer in bladder. The health condition can also affect the people in a same family or it can also be affected by age factors and sex. If a person is suffering with this cancer , he/she can have blood in the urine or a feeling of frequent urination. The person can also have pain in the urination process. However, this condition can also occur in normal health status.

The diagnosis process for a bladder cancer can be done with a urine test, it is also recommended as a pathology test. However, this test cannot be enough to understand the condition. The diagnosis process also includes radiology, it is also known as Intravenous Pyelogram or IVP Test. In this test, the condition of the bladder is checked with X-ray beams. This test helps the medical experts to understand the condition of kidney, bladder, urethras and other parts of the urinary tract area. The other test process will be a CT Scan or Computer Tomography Test. The Cystoscopy, is a type of test where the urologist have a direct visual of the bladder and urethra. The Magnetic Resonance Imaging or MRI is another method of test. This test is very vulnerable and provides in-depth idea of the bladder condition.

Bladder cancer has become a common issue and it can also attack small children due to a hereditary reason. Many Researches and Studies are going on to find other reasons and conditions for this cancer. However, the treatment process currently involves the chemotherapy drugs to ease the pain and prevent a further enhancement of the cancer. The other treatment includes radiation therapy. In this process, the radiated beams are thrown on the affected parts of bladder. Recently, study was done to identify the affects of Laparoscopic cystectomy in the treatment process. Medical experts have observed that the types of PDT or Photodynamic Therapy, in which the physician gives a non-toxic injection, can be supportive to give a significant halt in the tumor. A method of DNA change can also be able to cure a bladder infection or bladder cancer. Researchers are working on to find out the type of genes that can lead to a cancer or tumor in the bladder. Immunotherapy can also be a great help in the treatment process. It is a process that helps the immune system to wrestle with outer toxins and make the body metabolism strong. In this way, a bacteria, which is known as BCG or bacillus Calmette-Guerin, is inserted inside the bladder to make it strong and attack cancer cells. In the countries like the US, bladder cancer has been recommended as the eighth-most well-known health calamity. Physicians also have observed the patients who were given a combination of chemotherapy medicines, can have a great halt in the cancer. This set of medicines are known as MVAC medicines. The medicines that are used here are Docetaxel or Taxotere, paclitaxel or Taxol, Cyfos, Ifex and Gemzar. A resource has mentioned that, a latest clinical practice has found that if a patient is given the MVAC medicines in a proper manner, it could show a positive result on the cancer. Currently many of the massage therapy with herbal and essential oil, diets full of vitamins and much-needed minerals and even meditation has been proven as a significant heal for a bladder cancer.

The fact that some of the most common bladder cancer symptoms that many people experience are nonspecific, it means that the symptoms that a large number of individuals experience from this type of disease are symptoms that can also be seen in a wide variety of other types of illnesses and health conditions that do not have a thing to do with this particular form of cancer. Some of the most common that Some of the most common symptoms that individuals experience when they have this serious type of disease are as follows:

During urination the person may experience burning or pain that is not related to having a UTI or urinary tract infection.

The normal habits of the bladder change. In many cases the individual feels a strong and annoying urge like they need to urinate, yet when they go there is not very much urine produced. Some people notice that they need to urinate much more than usual.

Hematuria or blood found in the urine.

Generally the first warning sign of the bladder cancer symptoms that most people experience is blood that is in the urine. However, more often than not, you are not able to see the blood with the naked eye, and it is commonly known by physicians as microscopic hermaturia. The way that this problem is found is by having the individual produce a urine sample. If enough blood is present in the urine, there are times that some people may notice a slight change in the color of urine they produce, and may be seen as having either as an orange or pink hue

Often Times Bladder Cancer Symptoms are not noticed initially

One of the biggest problems with this specific type of disease is that in many cases the individual will not notice any type of warning signs that there may be something wrong with their health until the Bladder cancer has reached advanced stages, which often makes it very hard for physicians to cure. One way that people can help to prevent this serious situation from occurring, is to make it a point to have your family health care provider perform routine tests for screening. This is especially important in people that have risk factors that make their chances of acquiring this disease much higher.

Ways to Catch This Disease Before You Notice Bladder Cancer Symptoms

Aside from undergoing screening tests on a regular basis at your family physician’s office, it is also very important to contact your family health care provider if you notice that your urine has changed from its natural color. You will also want to make an appointment as soon as possible if you begin to notice any of the warning signs and symptoms that are most commonly associated with this type of serious disease. The sooner that a physician is able to adequately diagnose the condition that you are suffering from, the higher the chances will be that an effective treatment method can offer helpful benefits for the condition.

The first thing that you will want to do to ensure that you receive the best bladder cancer treatment that is available is to find a specialist such as a professional urologist that you feel the most comfortable with. This may involve consulting with a few different physicians before you feel comfortable in selecting the right doctor for your care. Then you will have an opportunity to discuss treatment options and ask any questions that you may have.

The Beginning Process of Your Treatment Plan

Your physician will explain each of the options that are available to you, including a description of the advantage and the disadvantages that are involved with each procedure. The specific bladder cancer treatment that you will receive will depend a great deal on a variety of different factors that your doctor will obtain while he reviews history of your condition, as well as your family history. Below is a list of the factors that will help your urologist determine a treatment plan that is right for your specific situation.

The overall condition of your health

Your age

Whether or not this is a recurring condition that you have suffered from in the past

The specific type of cancer that you have

The stage that the cancer is in

Using the professional advice of a few other health care professionals, your doctor will discuss the proposed form of treatment that he believes to be of the most benefit to you. Although the final decision is up to you, the proposed options that your physician may suggest will also be discussed with members of your family and other individuals that will be on the team that is responsible for your care.

Some of the Bladder Cancer Treatment Options That May be Used In Your Care

Below are some of the most common methods that are used in combination of one another for the treatment of this type of disease, and of course the sooner it is treated the better the patient’s chances for a cure will be.

Chemotherapy

Radiation therapy

Surgery

Biological therapy

Immunotherapy

In many cases, there will also be a dietician, nurses, a social worker, and other types of specialized professionals that will on your care team. Before any of these procedures are performed for your bladder cancer treatment, it is important that you thoroughly understand each of the aspects that surround the procedure. Your physician will explain what is involved and how each procedure will be performed; he will explain the benefits that will be involved, the advantages of the procedures, and the disadvantages that may also be involved with the use of each procedure. Many people find it to be much easier and much more comforting if they have a strong support system of friends and family members around them while they are going through bladder cancer treatment. A good support system in many cases can be very beneficial to an individual’s recovery and care.

If this serious and sometimes painful type of disease is caught in time, a specialized physician will be able to cure the disease fairly easily by using various types of effective treatment methods that are available. However, when this disease is in its early stages, it is common for most individuals to not even notice any symptoms of bladder cancer. Because of this problem, it means that many times the cancer has progressed into its latter stages before it is detected.

Areas of the Body This Disease May Affect

This form of cancer usually starts within the lining of the organ that is known as the bladder, and it can also make its way through the lining and then grow on into the muscles of the wall of the bladder. Invasive bladder cancer will often also spread into various other organs of the body that would include the lungs and the liver, the lymph nodes, and in the area of the individual’s pelvis. Once this occurs it can begin to cause them various problems with the function of the bowels and kidneys .

Some of the Most Common Symptoms of Bladder Cancer

When a person begins to experience some of the symptoms of this disease, it may include urination that is very painful, as well as blood being located within the urine. Below is a list of more of the warning signs that many individuals experience when they have acquired this dangerous disease.

Repeated problems with UTIs or urinary tract infections

Dysuria, commonly known as pain that may be experienced during urination

The need to urinate frequently and only producing a small amount of urine

Hematuria, which refers to blood clots or blood found in the patient’s urine. This is a serious problem that occurs in as many as 90% of all patients that suffer from the symptoms most commonly associated with this disease.

Common Symptoms of Bladder Cancer in More Advanced Stages

A growth or pelvis mass that can be found in the patient’s pelvis, close to the area of the bladder

Swelling that generally occurs in the patient’s lower leg area

Flank pain that is generally experienced in the area of the kidneys (lower back area of the patient)

When this serious disease has spread within the body, there are also some other types of symptoms that the patient may experience and would include the following:

Anemia

A loss of weight

Pain that may be experienced in the bones, the pelvic area, or the rectum

One of the most frustrating things about this form of cancer is that the symptoms of bladder cancer will often resemble the symptoms that are also associated with other types of conditions of the bladder. This is a problem that can delay an adequate diagnosis of the disease, which also means that there will end up being a delay in early effective treatment that can be extremely important for the overall health of the patient.

Gall bladder cancer diagnosis & tests

The doctor would conduct a detailed examination of the patient that would involve getting a comprehensive picture of the overall health condition and any past medical conditions. Blood samples would be drawn for checking the general health and the functioning of the liver. Several commonly employed tests for diagnosis of gall bladder cancer are:

Ultrasound Scan
During the test, sound waves are utilised for capturing images of the gall bladder and neighbouring organs. The patient is requested to not consume any solids or carbonated or milky consistency liquids and to solely sip clear fluids for four to six hours prior to the scan. The patient is told to lie down flat with the back against a comfortable surface. A gel is evenly applied over the abdominal area and a tiny microphone-alike appliance is then moved over the gel-covered surface. The sound waves produced are transformed into images with the assistance of a computer. This pain-free procedure lasts for fifteen to twenty minutes.

CAT or CT or Computerised tomography Scan
A CT scan captures a sequence of X-rays that are passed into a computer for building a 3-D imagery of the interior areas of the body. The scan is pain-free and lasts from ten to thirty minutes. CT scans employ miniscule quantities of radiation that would cause no harm to the person undergoing the test or to others who would be coming in contact with the patient. The person would be requested not to consume solids or liquids for at least 4 hours prior to the scan.

On majority of the occasions, the person undergoing the scan would be offered a drink or shot for allowing certain areas to be viewed more lucidly. Prior to having the shot or the drink, it is vital to inform the person performing the test regarding whether one is having any form of allergic reactions to iodine or is asthmatic. Subsequent to the scan the person could return home.

Spiral CT scan
A spiral or helical CT scan is a novel form of CT wherein the x-ray device continually revolves around the body of the person as it follows a spiral route for making cross-sectional images of the body.

MRI or magnetic resonance imaging scan
This test is analogous to a CT scan but employs magnetic fields rather than x-rays. During the course of the scan, the person would be required to be totally motionless while lying within a metallic cylinder. The cylinder is a high-power magnet; hence prior to being seated inside it, one would need to take off all forms of metallic objects one might be carrying along. It is imperative to inform one’s doctor whether one has previously ever been working in an industry that is metal-related or have personally ever worked with metals and is there any kind of metal present in the body like a cardiac monitor, pacemaker, bone pins, operative clips or others. An MRI cannot be done in case one has any of these metallic implants present in the body due to the magnetic fields emitted from them.An injection would be administered to the person for facilitating better viewing of the interior parts of the body. The test lasts for thirty minutes and is pain-free, though the device could sound quite loud for which the person would be offered ear muffs or headphones. A dual way intercom facility would enable the patient to converse with the person in charge of the scanner.

Endoscopic retrograde cholangio-pancreatography or ERCP
During this procedure an x-ray image is taken of the pancreatic and the bile duct. In case required, it could additionally be employed for unblocking the bile duct and for inserting the stent. No eating or drinking is allowed for nearly 6 hours prior to the test in order to clear out the stomach or the initial section of the small bowel (duodenum). A pill or shot could be given for relaxing – a sedative and a local anaesthetic spray would be used for de-sensitizing the throat. A fine pliable tube called the endoscope would then be passed via the mouth inside the stomach and the duodenum just ahead of it.By peering down the endoscope the doctor would be able to locate the aperture where the bile duct and the pancreatic duct are draining into the duodenum. A dye that would appear on the X-rays would be infused inside these ducts and the doctor would be able to clearly view any anomalies or obstructions.

Angiogram
The bile duct being located quite near the vital blood vessels of the liver, an investigative procedure known as the angiogram could be conducted for checking if there is any effect on the blood vessels due to the tumour.A delicate tube is introduced within an artery in the groin and a dye is infused via the tube. The circulation of the dye in the arteries facilitates better depiction on the x-rays. At times an MRI scan could be performed for revealing the blood vessels of the liver, after which there is no need for the angiogram.

Laparoscopy
This small operative procedure allows for easier viewing of the gall bladder, liver and other internally placed organs in the region around the gall bladder. This is performed under the influence of general anaesthesia and would translate to briefer hospital stays.Once the person is anaesthetized, a tiny slit would done by the doctor in the front of the abdominal region and a fine mini pliable, illuminated tube attached with a camera (laparoscope) would be inserted. This now makes it easier for the doctor to clearly view the gall bladder and remove a tiny tissue sample for microscopic evaluation (biopsy procedure).During the surgery, the abdominal cavity is pumped with carbon-dioxide gas that could cause discomforting wind and at times cause shoulder pains for many days on end. The pain is mostly allayed when one takes walks or has gulps of peppermint-infused water. Subsequent to the laparoscopy, 1-2 sutures would be done on the abdomen. At times during the course of a laparoscopy, there could be the removal of the gall bladder for treating gallstones or chronic inflammation affecting the gall bladder. The surgery is known as laparoscopic cholecystectomy. In case gall bladder cancer is diagnosed or is doubted during the surgery, the surgeon would alter the surgery to an open cholecystectomy wherein the gall bladder is removed via wider incision done in the abdomen. This would facilitate total removal of the cancer.

Laparotomy
In case the doctor is unable to make the detection using the above mentioned tests then a procedure known as a laparotomy could be conducted under the influence of general anaesthesia. During the procedure, an incision is made on the abdomen to allow easier examination of the gall bladder and the tissue adjacent to it for the presence of cancer. In case cancer has been diagnosed, but appears to not have metastasized to the neighbouring tissues, then the surgeon would be able to remove the cancer or reduce the obstruction it could be causing.

Gall Bladder Treatment

The kind of treatment offered is dependent on several factors inclusive of general health, the placing and the size of the cancer present in the gall bladder and has the cancer metastasized to other regions in the body.

Consent (Permission)

Prior to undergoing any form of treatment, the doctor would provide complete details regarding what one could expect and elucidate the objectives of the treatment being decided for the patient. The person would generally be requested to give a signature on a document that says that one is giving his/her consent or permission to the hospital staff to conduct the treatment. Without the person’s consent, no medical treatment could be offered.

Pros and cons of the treatment

Treatment could be offered for varying reasons and the potential advantages would differ in each case. In case one has been suggested a treatment which targets at curing the cancer, it is not that difficult to agree to such plan-of-action. But, in case the cure is not likely and the treatment is being suggested for controlling the cancer for a span of time, then it could be tricky for the person to come to a conclusion on whether or not to undergo the treatment.

In case one is undecided regarding the treatment when firstly elucidated, one could always request further time in order to make up one’s mind.

One is free to decide not to undergo the treatment and the hospital staff would expound what one could expect in case one does not go in for the treatment. No reasons are necessary to be given when one does not desire to undergo treatment, though it could be beneficial to let the staff know about one’s apprehensions so that they could offer the best possible advice on the same.

Surgery

Surgery is the key treatment in case of gall bladder cancer and could be used for removal of all the cancer existent in case it has not metastasized past the gall bladder.

In case the cancer has reached beyond the gall bladder, then surgery could still be beneficial for the individual in improving the arising symptoms by removal of majority of the cancer- inflicted parts.

In case surgery could be done or is not dependent on the outcome of the examinations illustrated above then in such a situation the person could be referred to a surgeon who has a keen interest in this atypical cancer.

Radiotherapy

Radiotherapy is used for the treatment of cancer by utilising high-power x-rays for obliterating the cancerous cells while at the same time doing minimal damage to the neighbouring healthy cells. This is infrequently employed for gall bladder cancer. It might optionally be administered externally with the assistance of a radioactive machine or a radioactive substance could be placed within the body in close proximity to the tumour.

Chemotherapy

Chemotherapy employs cancer-fighting or cytotoxic drugs for destroying the cancerous cells. They function by interrupting the spread of the cancer cells. Chemotherapy has been observed to not provide very effectual outcomes for treating gall bladder cancer. Though, it is considered to be beneficial in curbing the cancer for some span of time for the kind of gall bladder cancer that has metastasized in another place in the body.

Stent Insertion

In case the cancer present in the gall bladder is causing an obstruction in the bile duct then it could be likely that the doctor would introduce a fine tube also called as stent during the ERCP procedure. This could assist in allaying any jaundice with no requirement for surgical intervention.

The stent measures nearly five to ten centimetres in length and having the thickness of an ink pen refill. The stent helps in opening the route through the bile duct thus allowing the bile in draining away. The grounding and process is analogous to that of ERCP. By observing the x-ray imagery the doctors would be able to notice the constriction in the bile duct. The constriction could be dilated by making use of the inflatable balloons or dilators and the stent could be introduced via the endoscope for enabling the bile in draining.

The stent generally requires to be changed in a time interval of every three to four months to avert it from becoming clogged-up. In case that it does clog-up, then jaundice that at times might be accompanied by high temperatures could occur. It is imperative to inform one’s specialist regarding these signs on the earliest possible. A treatment course of antibiotics might be required in such a situation and the specialist could suggest the replacement of the stent with a new one. In majority of the individuals such a procedure could be done with relative ease.

Cancer affecting the gall bladder is an atypical occurrence with merely around five hundred individuals being detected with this kind of cancer in the United Kingdom annually. It is rarely found in individuals below fifty years of age and is mostly observed in individuals past the age of seventy. It is prevalent in the female gender more than in males.

Causes and Probable Risk Factors

The reason behind majority of the gall bladder cancers is unclear. There are varied risk factors that could raise the likelihood of developing this form of cancer that include:

Gall stones and Inflamed gall bladder
Gall bladder cancer more often occurs among individuals that have a past case of gallstones, or among those who have an inflamed gall bladder or cholecystitis. But, majority of the individuals having gall stones or an inflamed gall bladder would not necessarily develop gall bladder cancer.

Polyps
Benign tumours or polyps of the gall bladder would raise the risk of gall bladder occurring.

Anomalous Bile Ducts
Those that are born with congenital anomalies in the bile ducts have a slightly greater regularity of developing gall bladder cancer.

Presence of Porcelain Gall Bladder
Those having a condition known as porcelain gall bladder wherein the calcium depositions occur in the wall of the gall bladder, additionally raise the risk of developing this form of cancer.

Smoking
There is some proof indicating that those who indulged in cigarette smoking are more prone to developing gall bladder cancer.

Family History of this form of cancer
Those having close relations like parents, brothers or sisters who have gall bladder cancer are 5 times more prone to developing this form of cancer, though the level of risk is still quite less.

Being Obese
Those with weight issues or are obese have known to be at greater risk of contracting several cancers inclusive of gall bladder cancer.

Gall Bladder Symptoms

Gall bladder cancer in its initial stages is mostly asymptomatic and is generally found unpredictably when one is undergoing an operation for removal of gallstones. Nearly one in five gall bladder cancers are detected in this manner. Majority of the gall bladder cancers are only detected while they are in their later stages. They could lead to a host of symptoms namely:

Feeling sick and fever.

Abrupt pain in the upper right-hand side of the abdominal region which is the cavity that contains organs like the stomach, gall bladder and liver.

In case, the cancer obstructs the bile duct it could impede the movement of bile into the small bowel leading to reverse flow of the bile back into the blood and the tissues thus resulting in yellowing of the skin and the white parts of the eyes – a condition known as jaundice. The urine would appear a darker tone of yellow and stools would turn clay-like in appearance. The skin could turn scratchy.

Slight discomfort could be sensed in the abdomen, depleted appetite and reduction in weight might additionally take place.

Though these symptoms could be due to the presence of other conditions like gallstones or some gall bladder infection, it is imperative to get them evaluated by a doctor.

Gall Bladder Staging

The staging of the cancer depicts the extent and if it has metastasized past its initial location. By identifying the precise type and staging of the cancer, assists doctors in deciding the most apt line of treatment.

Cancer could metastasize either in the bloodstream or via the lymphatic system. The lymphatic system comprises of a mesh of lymph nodes interlinked by infinitesimal ducts containing lymph fluid that assist the body in combating infection and disease. The staging of the cancer is analysed by generally looking at the closely placed lymph nodes for finding the staging of the cancer.

Gall bladder cancer is present in four stages namely:

Stage 1
Nearly one in four cancers is detected in this stage wherein the wall of the gall bladder is affected.

Stage 2
The cancer has metastasized all the way through the thick wall of the gall bladder, however not reached the closely placed lymph nodes or adjoining organs.

Stage 3
The lymph nodes placed near the gall bladder or the liver, stomach, colon or the small bowel have been affected by the cancer.

Stage 4
The cancer is deeply seated in 2 or more organs located near the gall bladder or have reached the remotely placed lymph nodes or organs like the liver or the lungs. This is called as metastatic or secondary cancer.

A different TNM system is at times employed wherein:

T
Denotes the tumour size.

N
Denotes if the cancer has metastasized to the lymph nodes.

M
Denotes if the cancer has metastasized to another region of the body like the liver – known as secondary or metastatic cancer. Despite the complexity of this system, it could offer more accurate data regarding the staging of the tumour. In case the cancer relapses subsequent to the first treatment, it is called as recurrent cancer.

Grading

Grading implies how the cancer cells appear microscopically and provides information on how rapidly the cancer could be developing. Low grade translates to the fact that cancerous cells appear alike normal cells; they generally slowly proliferate and are less prone to spreading. In the case of high-grade tumours, the cells appear anomalous and have a likelihood of growing more rapidly and spreading.
