Early detection of any cancer types helps in achieving a better prognosis. The same stands true for esophageal cancer. For diagnosing esophageal cancer at an early stage, you must be aware of the possible signs of the condition. However, depending on the type of esophageal cancer one is suffering from, the symptoms of the disease can vary. Esophageal cancer type is determined based on the location of tumors; doctors ask the patients to get their cancer cells analyzed at the laboratory for identifying the esophageal cancer type they are suffering from. Esophageal cancer can be primarily of two types:

Squamous cell esophageal cancer:

In this type, the squamous cells i.e. the cells forming the lining of the esophagus get affected by cancer. This is the most commonly diagnosed esophageal cancer type in the world. However, in USA squamous cell esophageal cancer accounts for below 50% of all the cases of esophageal cancer. This is because risk factors of the other esophageal cancer type i.e. adenocarcinoma is more prevalent in America.

Adenocarcinoma:

As mentioned before, in USA this is the most commonly diagnosed esophageal cancer type. Adenocarcinoma occurs in lower portion of one’s esophagus or the portion of esophagus that is closest to the stomach. Gastroesophageal reflux disease or GERD is a risk factor for esophageal adenocarcinoma. GERD is a condition marked by traveling of stomach acid into esophagus resulting in the damage of the organ. This results in a condition known as Barrett’s esophagus; the damage caused to the esophagus increases the patient’s susceptibility towards getting adenocarcinoma.

There can be some other forms of esophageal cancer; but those types occur extremely rarely. They are small cell carcinomas, spindle cell carcinomas, lymphoma and sarcoma.

Consumption of processed and red meats has been linked with increased risk of developing colorectal cancer since a number of years. Recently, a study has also found links between red meat consumption and stomach and esophageal cancer.

This new study was conducted by a team of researcher under Amanda J. Cross. In this study, health of as many as 494,979 Americans, each between the ages of 50 years and 71 years were examined for 10 years. The participants of this medical study had to complete a questionnaire before the actual onset of the research. This questionnaire had queries regarding the nutritional habits of these participants, the process in which they used to cook or prepare their meat, their lifestyle (exercising habits, smoking habits etc), their height and body weight etc.

Cooking on high temperature generates HCAs or heterocyclic amines, many of which are categorized as carcinogens. The period of study that is the 10 years’ time resulted in development of esophageal squamous cells carcinoma in 215 participants, esophageal adenocarcinoma in 630 participants, gastric cardia adenocarcinoma in 454 participants and gastric non cardia adenocarcinoma in 501 participants.

It was found that consumption of red meat is positively linked with the occurrence of esophageal squamous cells carcinoma. Out of the 215 patients diagnosed with this condition, 69 occurred in participants who were in the top 5th as far as eating red meat were concerned. On the other hand, only 28 patients diagnosed with esophageal squamous cells carcinoma belonged to the bottom 5th of participants eating minimum red meat.

The researchers involved in the study however, state that the findings of their study do not establish red meat as a trigger of stomach and esophageal cancer; according to them, the study is only suggesting a possible link between consumption of red meat and stomach and esophageal cancer.

Munching on strawberries might well be a means of helping individuals having risk for esophageal cancer shield their bodies from it, as per a novel trial conducted by scientists from the Ohio State University.Comprehensive Cancer Center.

Doctor T. Chen who helmed the trial did a presentation of the outcomes during the AACR press gathering which is the foremost joint Ohio State cancer scientific study to be carried out in China. He stated that their trial team arrived at a conclusion 6 months of strawberry therapy is risk-free, easily consumable and their initial information indicates that strawberry lowered histologic grading of pre-malignant lesion formations & lowered cancer-linked molecular episodes.

Earlier printed study by this team noted that intake of strawberry after freeze-drying considerably restrained tumor growth in the esophageal region among rodent models. On the basis of such outcomes, the scientists got on a Phase 1b scientific study held in China for investigating how consuming freeze-dry strawberry affected pre-malignant lesion formation in esophageal cancer sufferers.

The team noted that everyday intake of strawberries lead to suppression of varied biomarker types that partook in tumorigenic activity in the esophagus inclusive of cell propagation, inflamed condition & gene transcription, the lead author stated.

All the thirty-six trial entrants consumed sixty gm. (around 2 oz.) of strawberry post freeze-drying everyday for 6 months. The scientists acquired biopsy samples pre & post intake of strawberries. Outcomes revealed that twenty-nine from thirty-six entrants sensed a lowering in histologic grading of the pre-malignant lesion formations in the course of the trial.

The trial group forecasts that most of the patient types having pre-malignant lesion formations in their esophageal area would be developing cancer of the esophagus in the due span of the forthcoming years. Their trial holds importance since it demonstrates that strawberry might slacken the advancement of pre-cancerous lesions in the esophageal region. Strawberry therapy might be an able substitute, or might be working in concert with other chemo-preventative medications, for preventing cancer of the esophagus. However, Chen adds that testing this in RCTs imminently is necessary.

Esophageal cancer holds 3rd spot for the commonest GI cancer & 6th slot for being the most common reason for cancer mortality worldwide. The trial group are researching ESCC (esophageal squamous cell carcinoma) constituting ninety-five per cent cases of esophagus cancer across the globe. Patient numbers surviving this form of cancer are quite dismal with merely ten percent of sufferers managing to live five years post-identification. ESCC is rather pervasive in Japan, some parts of S. Africa, China, France, Puerto Rico, Iran.

Chen has the biggest populace worldwide & the numbers of likely entrants for the scientific study is more as compared to any other nation worldwide.

In European region, Canada & the United States, esophageal cancer risk factors entail those using alcohols & tobacco, alongside pitiable dietetic intake short of fruit & veggie varieties.

In Oriental Region, added risk factors entail nutritional consumption of salted foods & food forms having varied mycotoxin contaminations, deficits in nutritional vitamin, mineral forms & thermal injury cases from intake of scalding drinks.

Over sixteen thousand newly surfaced esophageal cancer cases would be identified in the U.S. in 2011, as per distressing stats put forth by the ACS.

Esophageal cancer is classically carcinoma arising from the epithelial lining of esophagus and mostly being either adenocarcinomas or squamous cell carcinomas.

Although esophageal cancer pervasiveness in the U.S. isn’t as much as countries in Asia & Africa, yet mortality rates always surpass the thousandth score yearly in just the U.S. In case medical approximates put forth by the United States scientists are to be considered then the lifetime risk linked to the condition is one: two hundred. In the last ten years, though, therapy for this disease has shown significant improvement that has then lowered the numbers of esophageal cancer sufferers.

Esophageal Cancer Survival Rate

Figures for esophageal cancer survival rate are quite dismal. The cancer cells begin growth within the esophageal area & then metastasizing outwardly. Because of this reason, the esophageal cancer survival rate starts declining with each passing staging as the sickness heightens in severity. On the whole, esophageal cancer arises in four stages and survival probability of the disease dipping with patients crossing stages. Factually, in the final stage, survival rate is nearly zilch.

The tabular form mentioned herewith depict survival rate according to stage and their characteristic features.

Stages of Esophageal Cancer

Characteristics

Relative Five Yr. Survival Rate

0

Cancer cells are restricted to the deepest epithelial layer.

Seventy percent

I

Cancer cells assault transcending the epithelium and permeating the submucosa.

Sixty percent

IIA

Cancer cells metastasize, attacking muscle sheet of esophagus & also adventitia (in most probability).

Forty percent

IIB

Cancer cells metastasize past the epithelial layer, though not yet invading adventitia.

Twenty percent

III

Cancer cells invading the adventitia & metastasizing the adjoining lymph nodes or also the closely placed organs.

Fifteen percent

IVA

Cancer cells spreading to remote lymph nodes.

Fifteen percent

IVB

Cancer cells have reached remote lymph nodes &/or other organs.

Lesser than five percent.

The AJCC formed TNM system is deployed for grouping esophageal cancer into varied staging.

In case esophageal cancer diagnosis is in its nascent stages, survival prospects cross sixty percent, however with diagnoses being postponed, survival rate dips steadily. In comparison, stage III survival rate plummets under twenty percent which makes survivals nearly unattainable. In the table just given, stage four of the disease is segregated into A & B. In the first scenario, cancer metastasizes to remote lymph nodes, however yet there’s fifteen percent survival likelihood. In the 2nd case, though, the cancer cells metastasize to the lymph nodes and organs making the disease graver & survival prospects dimmer. Stages three & 4A has the analogous survival rate, though this does not translate to the fact that they offer any favourable depiction of the person’s health that continually shows rapid deterioration.

The esophageal cancer stats reveal that the male gender has greater vulnerability to the condition as compared to the female gender. Moreover, esophageal cancer occurrence augments as one ages, is seen to peak among individuals from the age band of fifty-five to eighty percent. The avg. survival rate for this disease (all staging coalesced) sums up to seventeen percent – is undoubtedly something which concerns medical circles globally.

When an individual has acquired this form of cancer, there are often specific warning signs and esophageal cancer symptoms that they will begin to experience because of a malignancy that has been found within the tissues or lining of the individual’s esophagus. One of the biggest problems in being able to correctly diagnose this form of cancer is that the symptoms that are often experienced resemble those that also occur in medical conditions that are known as a hiatal hernia, Barrett’s esophagus, and GERD. Because of this, a thorough examination and testing must be done on the patient.

Problems with Swallowing

The medical term that is used to describe problems with swallowing is known as dysphagia, and it is one of the most commonly suffered esophageal cancer symptoms of patients that have this disease because of the narrowing that occurs within the esophagus. This condition makes it much harder for the patient to swallow food and it generally starts off with difficulty swallowing big pieces of food. As the disease becomes worse, it can even begin to become difficult for the patient to swallow liquids.

Excessive Production of Saliva

As it becomes much harder for an individual to be able to swallow, the body instinctively begins to produce more saliva to help make it easier for the esophagus to pass food down to the stomach.

Pain That Occurs While Swallowing

In cases where the cancer has grown large enough that it begins to block the individual’s esophagus, there are many patients that find it very painful when they try to swallow. Not only is the pain felt as the individual swallows, and after they try to swallow food, but it can often be felt if there are lesions that have been made in the esophagus and in the lining or tissues within the esophagus, from the spreading of the cancer. This is one of the esophageal cancer symptoms that can also cause the individual to experience pain that can be felt between the person’s shoulder blades and within the chest area.

Weight Loss that is Unexplained

There are a large number of patients that have experienced unexplained weight loss. This generally occurs without the person even trying to lose weight.

Heartburn

Several individuals experience a sensation of burning that can be felt in the throat and in the chest area.

Vomiting

Some patients will endure the vomiting of blood that can escape from the cancer of the esophagus, and there are times when they vomit food as well.

Hoarseness

Another one of the common esophageal cancer symptoms that a large number of individuals suffer from is hoarseness in the sound of their voice. Many times they will also experience the constant need to clear their throat.

If you have noticed that any of these bothersome and painful symptoms have begun to occur, it is important to contact your family health care professional as soon as possible. The sooner that this esophageal cancer can be accurately diagnosed, the sooner your doctor can put a treatment plan into action.

Esophageal Cancer is a serious disease that occurs when a tumor forms within the lining of the individual’s esophagus. Over time this type of cancer has the ability to grow all the way through the wall of the body’s esophagus. Once this has occurred, there are many times that the disease will spread to other areas of the individual’s body by traveling through their lymphatic system.

When a person has acquired this type of serious disease, there is a wide variety of symptoms of esophageal cancer that they may begin to notice.

The loss of weight

The irritation of heartburn

Swallowing that causes the person to suffer from pain

Undigested food items that are regurgitated

Swallowing that is difficult

Old blood that is noticed in the bowel movements

Food products become stuck inside the individual’s esophagus

Difficult in being able to swallow foods that are solid, and eventually having the same problem with liquids

Vomiting blood

When a person begins to notice any of these warning signs and symptoms of esophageal cancer, it is extremely important that the individual’s family health care provider be notified. There are various types of tests and procedures that a family physician will want to schedule, in order to be able to make the determination of an adequate diagnosis.

There are numerous times that individuals will simply disregard heartburn and problems with a sore throat as being nothing more than frustrating and bothersome. However, if you begin to notice that any symptoms such as these are becoming much worse, or they seem to be persistent, then it is very important to make an appointment to visit with your family physician. Although there are many times that these types of symptoms is nothing serious, it is much safer to make sure before you completely disregard them.

Procedures That May Follow Persistent Symptoms of Esophageal Cancer

Once your physician has determined that the symptoms you are suffering from are showing signs to be more than just a minor problem that you are experiencing, there are various types of medical procedures that your doctor will more than likely need to perform to accurately be able to diagnose the problem that is occurring. Below are some of the most common procedures that many individuals undergo that are showing some of the signs of this particular type of cancer.

Computed Tomography

Barium X-Rays

Endoscopy

If you are an individual that has been suffering from the signs and symptoms of esophageal cancer and these symptoms have required that you undergo the above procedures, it is important that you speak with your physician about what each one involves. You will feel more comfortable if you know what will be done and what the benefits are that each one will provide. Remember, the sooner that your physician is able to diagnose the problems you are experiencing, the sooner a treatment plan can be put in place that will offer you the most benefits.

The cancer affecting the esophagus is of two key forms: squamous cell carcinoma and adenocarcinoma, dependent on the kind of cells which have turned cancerous.

Squamous cell carcinoma
These occur in cells which line the upper 2/3rd portion of the esophagus.

Adenocarcinoma
Generally arise in the glandular tissue present in the lower portion of the esophagus. This type is treated in analogous manner, however is based on the details of each person’s case.

Esophageal Cancer symptoms:

Esophageal cancer in its early stages generally is asymptomatic. But, as the cancer metastasizes, symptoms arising could include:

Problems during ingesting with associated pain.

Extreme reduction in weight.

Pain felt in throat or back, in the area flanked by the breastbone and shoulder blades.

Gruffness or persistent cough.

Puking.

Bloody coughs.

Diagnosis & Tests:

In order to decipher the reasons behind the symptoms, the doctor would assess the patient’s medical history and carry out a detailed physical examination. The doctor might probably suggest undergoing a chest X-ray and other kinds of diagnostic tests. These diagnostic tests comprise of the following:

Barium Ingestion
Also known as esophagram, is a sequence of X-rays of the esophagus. The patient would be told to ingest a liquid that contains barium that is known to coat the interiors of the esophagus. The barium leads to variations in the shape of the esophagus that appear on the X-rays.

Gastro-esophagoscopy
Also known as endoscopy, is an inspection of the interiors of the esophagus and stomach employing a slender, illuminated tube known as the endoscope. Anesthesia for causing numbness is generally employed during the procedure. In case any irregular area is spotted, then the doctor would gather cells and tissues via the endoscope for a biopsy to be conducted on it.

Biopsy
Is the removal of a tissue sample for pathological evaluation in order to formulate a diagnosis.

In case esophageal cancer is diagnosed, the doctor would ascertain the staging or extent of spread of the disease. Staging is a meticulous endeavour at finding whether the cancer has metastasized and, if so, to which regions of the body. Being aware of the staging of the disease assists the doctor in deciding the appropriate line of treatment. Stated below are elucidations of the 4 stages of esophageal cancer.

Stage I
The cancer is present solely in the uppermost layers of the cells that line the esophagus.

Stage II
The cancer has spread to the deeper layers of the esophageal lining, or has metastasized to closely placed lymph nodes, though not to the other areas of the body.

Stage III
The cancer has had a deeper spread into the esophageal wall or has metastasized to tissues or lymph nodes close to the esophagus, but not reached the other regions of the body.

Stage IV
The cancer has proliferated to the other areas of the body. Esophageal cancer could proliferate to any region of the body inclusive of the liver, lungs, brain and even the bones.

Esophageal Cancer Treatment:

Surgery
Surgery is the most prevalent form of esophageal cancer treatment wherein the tumor inclusive of all or parts of the esophagus, closely placed lymph nodes and other tissues present in the region could be removed. The surgery for removal of the esophagus is known as esophagectomy. The remnant normal section of the esophagus is then connected to the stomach to facilitate ingestion. The opening in between the stomach and the small intestine could also be widened for allowing easy passage of the stomach contents into the small intestine. At times, surgery is conducted subsequent to conclusion of other treatments.

Chemotherapy
Chemotherapy involves the usage of intravenously administered cancer-combatant drugs that travel through the body and help in killing cancer cells. Chemotherapy could be coalesced with radiation therapy as the main treatment, rather than surgery, or for shrivelling the tumor prior to surgery.

Laser Therapy
It employs high-power light for obliterating cancer cells. It solely destroys the malignant cells in the treated area. Laser therapy is done for annihilating tumor cells and relieving an obstruction in the esophagus when the cancer could not be eradicated via surgery. The respite from the obstruction could assist in allaying the symptoms, particularly those faced when trying to swallow.

Photodynamic Therapy (PDT)
PDT is a kind of laser therapy involving the usage of drugs that are assimilated by the cancer cells. The specialised light when focused onto the malignant region causes an activation of the drugs absorbed by the cancer cells that leads to their destruction. PDT procedure could be employed for allaying symptoms of esophageal cancer like problems faced during ingestion.

Radiation therapy
It is also known as radiotherapy wherein the high-power X-rays are used for killing the cancer cells in the specific malignant area. The radiation could be emitted from a machine located outside the body, also known as external radiation, or come from radioactive materials that are placed in or close to the cancer cells, known as internal radiation. A plastic tube could be introduced into the esophagus for keeping it open at the time of the treatment. This treatment procedure is known as intraluminal intubation and dilation. Radiation therapy could be administered by itself or in combination with chemotherapy as main treatment rather than or prior to surgery, particularly when the size and placement of the tumor makes it tricky for removal during surgery. Even when the tumor could not be eradicated during surgery or totally obliterated by radiation therapy, still radiation therapy could mostly aid in allaying pain and make ingestion easier.
